calendar-year deductible, this plan offers a choice of
four daily maximums. Choose a daily-deductible maximum of
$250 for individual coverage and your expenses will not
exceed $250 per day for any covered expense. Plus you
still have a convenient $50 co-pay for a physician office
visit charge. All other covered medical services are
subject to the daily maximum.
